Bluetooth headsets: you can now talk through the headset's own microphone, not just hear through it. iOS only offers a Bluetooth microphone on the hands-free profile and the app was never switching to it, so the headset was output-only and you were talking into the phone. Choosing the built-in microphone in Settings keeps the higher-quality playback and uses the phone's mic instead.
