LynCare

LynCare is a private coordination app for self-directed home care. Self-direction puts the participant in charge — their care, their choice, their voice. LynCare is built around that: the participant, their caregivers, and their Care Manager all work from one clear, shared picture of what's been done and what's coming up. Built for participant-directed Medicaid care (Idaho's A&D and DD Waiver programs), LynCare brings shifts, medications, tasks, notes, and team communication into one secure place — replacing scattered texts, notebooks, and whiteboards. WHO IT'S FOR LynCare is used by an invited care circle: • Participants directing their own care • Caregivers they hire • Care Managers who oversee the arrangement Access is by invitation — you'll need an invite or access code from your Care Manager to join a circle. WHAT YOU CAN DO • Shifts — clock in and out, see who's on shift, and hand off at the end of a shift • Medications — keep scheduled (eMAR) and as-needed (PRN) administration records, with missed-dose tracking • Tasks — manage daily and recurring tasks across the whole team • Daily notes — log what happened during a shift and control who can see each note • Health events — record and notify the team of a mild, moderate, or severe concern • Team communication — share updates and broadcasts with the people who need them • Calendar — keep appointments and schedules in one place • Notifications — stay on top of medications, notes, and anything that needs attention PRIVATE BY DESIGN LynCare handles sensitive care information, so privacy is built in: per-note visibility controls, role-based access, on-device PIN lock, and a clear record of who did what and when. You stay in control of your circle's data. Access requires an invitation from a Care Manager.
