It seems that you have incorrect permission settings. You can tap the third tab (gear icon) in bottom of the app to open the "Settings" page. You can see "No alarm triggered?" item a little bit down and tap it. There are two items to check. If they are all set, both would have a green check mark on the side, otherwise tap and follow the instructions. If it does still miss, set the diameter to 1000m of the alarm to compensate the accuracy of GPS to the area. Hopefully, the map alarm working fine for you and I could get a positive feedback.

The accuracy of location calculation is based on the signal the device is receiving. It means that if your phone is at the place where are weak GPS signal and weak phone signal, it may generate inaccuracy location info due to this device limitation. For example, device in subway or indoors, places near tall buildings/mountain, or with bad weather, or far from any mobile signal tower. May I ask, what kind of target location at which the app is silent? For your case, if you are driving, I guess you could try setting larger alarm trigger area and place the phone near window for better signal receipt to improve the situation.
