Grateful for this app

This app helps my doctor get the info he needs to prescribe my medications and meet the state and federal requirements. I see so many bad reviews because patients are not happy they have to do a little extra work. If they only knew that, without this app, they probably wouldn’t get their meds, they may feel differently.

What is the point?

A clinic I went to wanted me to do this. First questionnaire and it is getting mad at me because I am not answering a question that is not appearing.

I have followed all of the steps to register, and I still cannot login

Worthless questions, boring answers

Forced to get this app from provider….guess you pay them for patients required filling this out.
None of these questions can accurately provide anyone with a poor idea over someone’s health. Each day is different and clicking a box can/will never describe how/what one experiences to correctly dictate how an individual should/can be medically treated.
I was shot when I was younger. Does that dictate how I’m going to feel, react, share, hide, fear, worry 30 years after the event? Maybe to some, but I don’t let it dictate how I live, or emotionally/physically feel. Drs over the years have misdiagnosed/under perform their duties just due to the fact an idiot wanted to steal my friend’s car, I spoke back, caught a couple scars, yet drs focused solely on medical books and training to inform me how my body decades later is going to physically perform. In doing such, they neglected asking further studies, missing neurological disease, not related at all to past years events…now being untreated for 20years, I am no longer able to control my legs, my left arm, permanent brain damage, permanent nerve damage, ongoing losing permanent control over parts of my body. Yet my woes or concerns are to be distracted by the use/not use of certain medications.
What ever, this app seems to overtake the misguided aspects of a medical book, written decades ago, by blanketing all people’s care under their very own personal life experiences under the same umbrella.
Really, really terrible…I thought I selected my current medical team better than relying them relying on outdated, proven incorrect over and over again.

Cannot open the questionnaire through the app

Poor app design. Cannot load questionnaire through the app. Had to log online both times. There’s also no way to contact customer support through the app. This does nothing but adds stress to the pain management programs.
