Great for other purposes too

I'm sure this works great for petitions, but I just used it at a major show as a way for people to sign a confidentiality agreement before viewing a product under NDA.

I think the app creators could make editing the fields a little more intuitive (for example, if you want to remove a text field, you have to delete all the default text...I think it would make sense to click an "X" to delete it.)

Overall, really handy solution...thanks!
