About

PolyMeds is a private medication record-keeping app built for caretakers managing more than one loved one's regimen. Most medication reminders assume the user is the patient. PolyMeds is built for the adult child, spouse, or family helper who needs one calm place for multiple medication lists, refill dates, dose history, prescribers, pharmacies, allergies, and doctor handoff notes. WHAT YOU GET - Multi-profile medication records: track up to four loved ones with separate schedules, notes, prescribers, pharmacies, allergies, and conditions. - Plain-English interaction review: PolyMeds checks each active list against public FDA drug-label interaction sections and NIH RxNorm identifiers, then shows source-backed summaries you can discuss with a clinician or pharmacist. - Refill ladder: see what is running low across the family before a weekend, holiday, or appointment. - Doctor handoff PDF: generate an on-device summary with active medications, dose history, allergies, care team details, and interaction notes. - Prescription label scanning: use the camera to start from label text, then confirm every field before saving. - Private by default: records live on your device and your private iCloud. Drug lookups send only drug identifiers or search text, never a loved one's name. FREE TIER Track one loved one and up to five active medications. POLYMEDS PRO Unlock up to four loved ones, expanded medication lists, interaction review, refill planning, caregiver missed-dose alerts, and doctor handoff exports. Monthly and yearly subscriptions are billed by Apple and can be cancelled anytime in iOS Settings. IMPORTANT PolyMeds is a private record-keeping tool, not medical advice and not a regulated medical device. It does not prescribe, diagnose, treat, or replace a clinician or pharmacist. Interaction notes summarise public clinical sources, including US FDA openFDA Structured Product Labels and US National Library of Medicine RxNorm. Always consult the prescribing clinician and pharmacist before changing any medication regimen. Privacy Policy: https://meds.uptrail.ai/privacy Support: https://meds.uptrail.ai/support
