The Smart System of Disease Management (SSDM) is a series of doctor-patient interactive applications developed for self-management of patients with chronic diseases. It is divided into "iRheuma " for patients and "Rheumatologist" for doctors. Patient side includes self-assessment (such as disease activity assessment DAS28, physical function assessment HAQ), drug administration, adverse event management and laboratory tests, etc. The interface is friendly and easy to operate. The team's research shows that rheumatic patients can accurately master SSDM and implement disease self-management. The papers were accepted by the American Congress on Rheumatism (ACR) at 2015 Academic Conference and were presented to rheumatologists and patients worldwide as presentations.
