Needs A LOT more work
First, there IS an in-app purchase requirement if you want more than the 8 free responses. That said, there is no way to subscribe or even find out how much the subscriptions are. This seems like it would be the most basic programming element — decide on $$, create a way to collect $$, make sure the App Store has those details.
