It is not a game. It is the list you keep beside you while you play.
THE ROUTE
Most checklists are a stack of lists and a search box.
This one opens on a road.
The whole run, left to right, in the order you walk it. Optional places hang off the area they are entered from. So do the two downloadable ones.
It knows where the game forks. The Cathedral of the Deep and Farron Keep both leave the Road of Sacrifices, and it says "either first" instead of picking one for you.
It knows which door opens which place. The statue for Archdragon Peak stands in the Irithyll Dungeon. The gesture that works it lies four areas on.
Wherever you are standing, one card says what to do there. The fight. The steps that matter here. What the area holds.

BEFORE YOU MOVE ON
Eight things in this game close a door behind you.
A merchant who gives up on you. A questline that ends the moment you do something else first.
The warning sits on the step that keeps the door open, while it is still open. "One more boss without a scroll and Orbeck leaves for good."
Not afterwards, when it is a receipt.
THE FIGHTS
25 bosses. Souls, weaknesses, drops, and two or three lines on how the fight actually goes.
Names and art stay hidden until you get there, so the road is safe to read on a first playthrough.
WHAT IS IN IT
Five trophy lists. 38 sorceries, 38 miracles, 30 pyromancies, 116 rings, 33 gestures. Master of Sorceries through Master of Expression.
Every ring counted the way the trophy counts it. Master of Rings asks for the upgraded versions too, so a +1 and a +2 are rows of their own, each saying where that version is rather than where the plain ring was.
Ashes of Ariandel and The Ringed City are listed and counted toward no trophy, because the game does not ask for them. A full ring reads full whether you own the DLC or not.
The item text as the game writes it. Not a paraphrase.
How each one is come by: bought from a merchant, transposed from a boss soul, paid out by a covenant, or lying on a corpse.
Every entry placed in one of 21 areas, from the Cemetery of Ash to the Ringed City.
HOW IT WORKS
Tap the circle to mark something found. An item, a fight, a questline step. The road moves with you.
Tell it which playthrough you are on and it says what is out of reach. A ring that only appears in New Game+. Something a decision has already cost you. It is a note beside the count, never taken off it.
Group any list by area and it folds into the places you walk through, in the order you walk through them. Fold away what you have cleared.
A progress ring per trophy, so you can see which one is closest. Each says what is left and which area holds most of it.
Search and filter inside any list. Still missing, already found, base game or DLC.
Progress syncs through your own iCloud account. It follows you to a new iPhone and comes back if you delete the app and reinstall it. No account of ours, no sign-in, no server of ours involved.
No ads. No tracking. No subscription. Everything up to and including Irithyll of the Boreal Valley is free, and one purchase opens the rest, once and for good.
