Updates and Bug Fixes: Bug in date selection for injury time was not working correctly in iOS 14 and 15. This bug has been fixed and the date / time selection has been modified to make it simpler to select both date and time.

App + Website no longer supported.

You need to email the PDF through the App, but that function is not working for me for some reason, even though the form is completed.

There is no ability to create and download a PDF of your report, so it’s just stuck in the app..

Please allow us to just download the PDF report so we can email it separately.

Great App for Coaches

I stumbled upon this app and find it easy to use. It pulls info from my team management app, which saves time when filling out an injury report. Simple way to track injuries for the team.

More injuries on app

I would like to see more sport injuries part of the app besides head even though it’s important. Like to see ankle, knee and just over all injuries. I think it’s important so we can keep track of all injuries no mater how small or big they are.

Our intent is to cover all types of injuries no matter how small they might be. The Incident Description section allows you to describe fully the injury and refer to it in the reports. Head injury protocols are very specific so those have been explicitly specified. If there are other - better - ways to specify injury types, we would enjoy hearing the ideas from our users. We continue with our goals of providing a free resource to help leagues and their players document injuries and submit in a timely manner to the overseeing league administration.

Little League Safety Officer

I am the Safety Officer for Folsom National Little League and we just implemented this app mid-season 2017. It is super easy to use and, although we hope to rarely need it, it makes the injury reporting process a snap. The UI is intuitive and the resulting report is very thorough and easy to read. I cannot say enough good things about this app!

Especially now, with CA concussion protocols in effect for all ages, this app is an essential tool in our toolkit to help keep our little athletes safe through timely and open communication of injuries on and off the field.
