Waste of time - input your records = unable to extract
After inputting all my 30 + vaccines from multiple paper docs and states, find it is impossible to print an excel, pdf of the self reported vaccines. The only way to export seems to be via a mystery vaccine code, not on any cdc or hhs document.

Never got an email to confirm after entering all the data on the app. Never got my test results, looked up their number on Google and it just rings.

Login/lost info
Go to login to show my card while on vacation in California, cannot log in, get logged in finally, and every vaccine entered from Veterans hospital is gone- where did my data go???
Waste of Time
I had the same issue as a reviewer in February (it is May). When I take a picture of my CDC Vaccine card, it says my birthday doesn’t match the records that I just submitted! The app has no Help link. Obviously the developer isn’t serious about this app. Move on!
App crashes when trying to enter patient data
When I click the button to enter patient data, the app crashes and closes.

I’m in richmond va. So how do I get my vaccine information on app? Would my medical provider have this information. Says medical profession has to sign in. So as a user. Can I set up acct and give to med provider to send me info?
Useless
I tried to enter my two COVID vaccines manually, kept changing the dates given to the day prior. Weird glitch? So I tried to enter them by taking a picture of my CDC vaccination record card to confirm the information. It kept giving me an error message after 6 different picture attempts that my name and birthdate did not match the information provided on the HIPAA form (which it clearly did). Just wasted a good 45 mins of my time and nothing to show for it... hopefully this review will save someone else's precious time. I need to find a new app to log this info securely for me.
