VentraLink: My Medications

VentraLink helps you keep one accurate, up-to-date list of everything you take, so you and your doctor's office are always looking at the same information. You have probably been asked to list your medicines at almost every visit. Even so, the list in your doctor's chart is often out of date, because what you take changes over time and small things are easy to miss. When your doctor's office uses VentraLink, the list you keep here is shared with their team, so they can see what you actually take and keep your record current. What you can do: See your current medicines, with the name, strength, and how often you take each one. Mark a medicine as still taking, stopped, or taken differently than written. Add a medicine, including over-the-counter products and supplements. Review and update your list before an appointment or after a hospital stay. How to get started: Your doctor's office will send you a text message with a sign-in link. Tap the link and follow the steps to confirm who you are. You need an invitation from a participating doctor's office to use VentraLink. Your privacy: We follow HIPAA privacy standards. You decide what is in your list. Your information is encrypted in transit and at rest. VentraLink does not give medical advice and does not replace talking with your doctor or pharmacist.
