It’s tremendously expensive
Update: many Apple Watch WhatsApp works now with cellular and without the need of paying any additional. There is no need of a cloud, I don’t know what is the developer saying here because other developer do offer the possibility of using it with WiFi or cellular

The basic functionality of Pairing mode, which requires an iPhone to work with Apple Watch, is free for life. You only pay for voice messages, quick replies, etc. Cloud mode is paid and relatively expensive because it works with a separate server, and we must pay for the server so that it can work without your iPhone. All data in Pairing mode remains only on your iPhone and Apple Watch. Data in Cloud mode is stored on the server and Apple Watch, and all data on the server is securely stored and deleted when you log out.

If you are referring to voice message, the recorded voice will be sent as audio message. You will need to grant permission for recording. If it doesn't work, uninstall and reinstall and you will see the permission window.
