Verdict
The clearest difference is review sentiment: Echo UW at 100% against STS Cardiothoracic Surgery's 50%. Echo UW also leads on monetization model (free vs free with in-app purchases) and iOS requirement (8.0 vs 15.0). STS Cardiothoracic Surgery's advantage is rating (4.8 vs 4.6) and ratings volume (205 vs 10). On price, ads, in-app purchases and device support there is nothing between them.

Both are free to download. Neither carries in-app purchases, so what you see is what you pay.
STS Cardiothoracic Surgery holds the better App Store score, 4.8 against 4.6. That said, the two scores are not equally well evidenced: STS Cardiothoracic Surgery has 205 ratings behind it against 10, so its average is the more settled of the two. Written reviews complicate that: 100% of Echo UW's written reviews are positive against 50% of STS Cardiothoracic Surgery's, so the app with the better star average is not the one people write more warmly about.
