Verdict
The clearest difference is in-app purchases: Island TV at none against Multi's 1. Island TV also leads on ratings volume (45 vs 2) and iOS requirement (16.6 vs 26.0). Multi's advantage is rating (5.0 vs 4.7). On price, ads and device support there is nothing between them.

Both are free to download. Multi sells 1 one-off in-app purchase. Island TV asks for nothing beyond the download.
Multi holds the better App Store score, 5.0 against 4.7. That said, the two scores are not equally well evidenced: Island TV has 45 ratings behind it against 2, so its average is the more settled of the two.
