This app is intended for use by health care providers in their diagnosis and management of chronic kidney disease (CKD). Recommendations included in this app are based on the KDIGO 2012 Clinical Practice Guidelines. Kidney Disease: Improving Global Outcomes (KDIGO) CKD Work Group. KDIGO 2012 clinical practice guideline for the evaluation and management of chronic kidney disease. Kidney International Supplement 2013;3:1-150. Because CKD is a chronic health problem, clear communication between provider and patient is needed to promote effective self-management by the patient.
