Verdict
The clearest difference is review sentiment: Notes on Watch at 100% against Watch+ Widgets & Wallpapers's 50%. Notes on Watch also leads on in-app purchases (none vs 2) and iOS requirement (14.0 vs 16.6). Watch+ Widgets & Wallpapers's advantage is ratings volume (712 vs 2) and rating (4.7 vs 4.5). On price, update cadence, ads and device support there is nothing between them.

Both are free to download. Watch+ Widgets & Wallpapers sells 2 in-app purchases, starting at $1.99 per month. Notes on Watch asks for nothing beyond the download.
Watch+ Widgets & Wallpapers holds the better App Store score, 4.7 against 4.5. That said, the two scores are not equally well evidenced: Watch+ Widgets & Wallpapers has 712 ratings behind it against 2, so its average is the more settled of the two. Written reviews complicate that: 50% of Watch+ Widgets & Wallpapers's written reviews are positive against 100% of Notes on Watch's, so the app with the better star average is not the one people write more warmly about.
Watch+ Widgets & Wallpapers ships an update every 5 months, Notes on Watch every 8 months. The most recent releases landed on October 10, 2026 and October 6, 2026 respectively.
