Verdict
The clearest difference is review sentiment: Qure.ai at 100% against Basic Radiology's 50%. Qure.ai also leads on in-app purchases (none vs 1) and iOS requirement (11.0 vs 15.1). Basic Radiology's advantage is rating (4.8 vs 3.0) and ratings volume (159 vs 2). On price, update cadence, ads and device support there is nothing between them.

Both are free to download. Basic Radiology sells 1 in-app purchase, starting at $9.99 per month. Qure.ai asks for nothing beyond the download.
Basic Radiology holds the better App Store score, 4.8 against 3.0. That said, the two scores are not equally well evidenced: Basic Radiology has 159 ratings behind it against 2, so its average is the more settled of the two. Written reviews complicate that: 50% of Basic Radiology's written reviews are positive against 100% of Qure.ai's, so the app with the better star average is not the one people write more warmly about.
Basic Radiology ships an update every 2 months, Qure.ai every 3 months. The most recent releases landed on September 30, 2026 and October 1, 2026 respectively.
