SOAPy: Clinical Note Trainer is the clinical documentation trainer built for medical students, nurse practitioner students, physician assistant students, residents, and practicing providers who want to write better SOAP notes — faster. Practice on detailed, realistic patient scenarios across multiple cases and medical specialties, including Internal Medicine, Emergency Medicine, Pediatrics, Psychiatry, OB/GYN, Neurology, Cardiology, Pulmonology, Orthopedics, Dermatology, Geriatrics, Infectious Disease, Gastroenterology, and Endocrinology. Every case includes a full patient history, vital signs, physical exam findings, labs, and imaging — just like real clinical practice. AI-POWERED ATTENDING FEEDBACK Submit your SOAP note and receive instant feedback from an AI "attending physician". Your note is scored section by section — Subjective, Objective, Assessment, and Plan — each out of 25 points. You'll learn exactly what you documented well, what you missed, and why it matters clinically. BUILT FOR HOW CLINICIANS ACTUALLY DOCUMENT SOAPy: Clinical Note Trainer uses a structured narrative format that mirrors real EHR documentation. Each section includes guided fields for chief complaint, HPI, PMH, medications, allergies, social history, family history, and a full review of systems — then lets you write freely in your own clinical voice. FEATURES THAT MAKE YOU BETTER Daily Case: A new scenario every day Attending Mode: Review a note full of errors and find them all — just like a real peer review AI Patient Chat: Ask the patient follow-up questions and get in-character responses, simulating a real clinical encounter Voice Dictation: Speak your note out loud exactly like providers do in real practice Weak Spot Drill: The app detects your lowest-scoring section and recommends targeted cases Note Comparison: See your note side by side with an ideal note after submission Timed Challenge: Practice under pressure with a countdown timer Score History: Track your improvement over time with a visual graph Dark Mode: Easy on the eyes during late-night study sessions PERSONALIZED TO YOUR TRAINING LEVEL SOAPy: Clinical Note Trainer adapts its feedback to your role. A first-year medical student receives guidance different from that of a PA student or a practicing nurse practitioner. Select your role and specialty at onboarding, and the app tailors every teaching moment to where you are in your training. WHY SOAP NOTES MATTER Poor documentation is one of the most common sources of medical errors, billing denials, and malpractice claims. SOAP notes are the foundation of every patient encounter — yet most programs give students only a handful of chances to practice before they are expected to write them independently in the clinic. SOAPy: Clinical Note Trainer gives you unlimited reps. Practice as many times as you want, on as many cases as you want, and get immediate expert feedback every time. Whether you are preparing for OSCEs, shelf exams, clinical rotations, or simply want to sharpen your documentation before your next shift, SOAPy: Clinical Note Trainer is your personal attending, available 24/7.
