This application started as a project required by school, to create a product about any of my interests. Taking the time to work on it, made me realize that it can be something more than that, a tool to achieve a bigger goal, and a method to help people. My passion for medicine has been growing for quite a while now, and it is my responsibility to share what I have learned with my community, and my people. PAH is a serious condition which takes a countless number of lives every year. Presenting patients with alternative treatments other than drugs was my way of leaving my touch and my voice. ~ Mahmoud Al-Mowaswess

Verdict
The clearest difference is rating: PAH at 5.0 against Dakar Rally's 4.1. PAH also leads on monetization model (free vs free with in-app purchases). Dakar Rally's advantage is ratings volume (153 vs 3) and device support. On price, in-app purchases and iOS requirement there is nothing between them.

Both are free to download. Neither carries in-app purchases, so what you see is what you pay.
PAH holds the better App Store score, 5.0 against 4.1. That said, the two scores are not equally well evidenced: Dakar Rally has 153 ratings behind it against 3, so its average is the more settled of the two.
