Verdict
The clearest difference is review sentiment: SubtleSTEMI at 100% against EM Coach's 50%. SubtleSTEMI also leads on in-app purchases (none vs 1) and iOS requirement (10.0 vs 13.0). EM Coach's advantage is rating (4.7 vs 4.2) and update cadence (every 5 weeks vs every 8 months). On price, ads and device support there is nothing between them.

Both are free to download. EM Coach sells 1 one-off in-app purchase. SubtleSTEMI asks for nothing beyond the download.
EM Coach holds the better App Store score, 4.7 against 4.2. That said, the two scores are not equally well evidenced: EM Coach has 113 ratings behind it against 9, so its average is the more settled of the two. Written reviews complicate that: 100% of SubtleSTEMI's written reviews are positive against 50% of EM Coach's, so the app with the better star average is not the one people write more warmly about.
SubtleSTEMI ships an update every 8 months, EM Coach every 5 weeks. The most recent releases landed on November 1, 2026 and November 2, 2026 respectively.
