Verdict
The clearest difference is in-app purchases: Piko's Blocks at none against Buttons and Scissors's 5. Piko's Blocks also leads on ads (none vs shown) and update cadence (monthly vs every 12 months). Buttons and Scissors's advantage is ratings volume (1.7K vs 6) and iOS requirement (12.0 vs 15.6). On price, review sentiment and device support there is nothing between them.

Both are free to download. Buttons and Scissors sells 5 one-off in-app purchases. Piko's Blocks asks for nothing beyond the download.
Piko's Blocks holds the better App Store score, 5.0 against 4.6. That said, the two scores are not equally well evidenced: Buttons and Scissors has 1.7K ratings behind it against 6, so its average is the more settled of the two. Written reviews complicate that: 82% of Buttons and Scissors's written reviews are positive against 75% of Piko's Blocks's, so the app with the better star average is not the one people write more warmly about.
Buttons and Scissors ships an update every 12 months, Piko's Blocks monthly. The most recent releases landed on October 14, 2026 and September 26, 2026 respectively.
