Doctor Visit Log & Tracker

Walk out of every appointment with the details already saved. Doctor Visit Log & Tracker keeps each doctor visit — who you saw, their specialty, the diagnosis, the prescriptions, the follow-up date — in one private log on your iPhone. No patient portal, no account, no clinic login. Portals show you what one clinic recorded, behind one more password. This app is the other half of your medical history: what the doctor actually said, in your own words, across every provider you see — the cardiologist, the dermatologist, the walk-in clinic that has no portal at all. WHAT YOU GET • Your next follow-up front and center the moment you open the app — date, doctor, and the note you left yourself • Every visit in one searchable timeline, so "what did Dr. Chen say about my knee in March?" takes seconds, not a phone call • Prescriptions and treatment plans written down before you forget them in the parking lot • A reminder the day before each follow-up, so the appointment you meant to book actually happens • Questions saved before you go in — tick each one off while you are still in the room • Your whole history as a clean text file for any new doctor — AirDrop it, email it, or save it to Files PERFECT FOR • Caregivers coordinating a parent's appointments across several specialists • Anyone managing a chronic condition with recurring follow-ups • Patients whose records are scattered across clinics that do not talk to each other • Anyone who has ever left an exam room thinking "what did they just say?" WHY NOT JUST THE PATIENT PORTAL? The portal holds the clinic's version, one provider at a time. This app holds yours: every doctor in one timeline, the plan as you understood it, and the questions you still need answered — available offline, even for providers with no portal. PRIVATE BY DEFAULT Your records are stored on your iPhone. No account, no cloud, no analytics, no tracking. Your data leaves the device only when you choose to export it. FULL ACCESS This app requires a purchase. Full access unlocks unlimited visit records and every tool above — the searchable timeline, follow-up reminders, question checklists and text export. Choose a weekly or yearly subscription, or a one-time lifetime purchase. Weekly and yearly plans are auto-renewable subscriptions. Payment is charged to your Apple Account at confirmation of purchase. A subscription renews automatically at the same price unless it is cancelled at least 24 hours before the end of the current period; your Apple Account is charged for the renewal within the 24 hours before the period ends. Manage or cancel a subscription in your Apple Account settings at any time after purchase. The lifetime option is a one-time purchase and does not renew. Log your last visit tonight, while you still remember what was said — your next appointment will thank you. Doctor Visit Log & Tracker is a personal record-keeper. It does not verify or interpret medical information and is not a substitute for professional medical advice, diagnosis or treatment.
