Log a migraine in one tap, even mid-attack, on a dark screen that won't hurt your eyes. Throb is a private migraine and headache diary you pay for once: no subscription, no account, no ads. ABOUT THROB Throb is a migraine and headache tracker for iPhone (iOS 18 or later) with an Apple Watch app. It is made for people with frequent or chronic migraine who need a reliable headache diary for themselves and for their doctor or neurologist. Everything is stored on the device; nothing is sent to the developer. LOGGING • Start an attack with one tap from the app, the Lock Screen widget, Control Center, the Action Button, Siri ("Log a migraine in Throb") or Apple Watch. • Attack mode is a dark, low-light screen. Tap the pain level as it changes to draw a pain-over-time curve, and log every dose. • Record where it hurts on a head map, plus symptoms, triggers, aura and prodrome, relief methods and notes. • Whole-day entries for daily or continuous headache, with no start or end time. • "Feeling one coming" notes early warning signs before the pain starts. • Log medication doses taken outside an attack, including preventives. FOR YOUR DOCTOR • Doctor report PDF: headache days, attacks, duration, peak pain, acute-medication days, how often each medication helped and its median time to relief, MIDAS disability score and the HIT-6 questionnaire. • A monthly count of acute-medication days, shown as information, never as an alarm. INSIGHTS • Calendar heat map, trends and a searchable list of every attack. • Co-occurrence: which triggers and symptoms show up with your attacks. Patterns only, never claimed causes. • Preventive treatment check: headache days per 30 days before versus since you started a preventive, with optional reminders from daily tablets to injections every few weeks. • Barometric pressure trend from the iPhone's own barometer, optional alerts on sharp drops, and an optional pressure forecast from Apple Weather. DATA AND PRIVACY • Works fully offline. No account, no analytics, no third-party SDKs, no ads. • Optional iCloud sync through your own private iCloud database. • Optional Apple Health: reads sleep and menstrual cycle, writes headache events. • Automatic local backups with restore, plus CSV and JSON export. • Import from a Migraine Buddy CSV export or a headache spreadsheet. • Face ID lock. PRICE Free: logging, attack mode, full history, calendar, search, backups, iCloud sync, export, Face ID lock and a doctor report for the last 30 days. One-time purchase, no subscription: doctor reports for any date range and all insights. LANGUAGES 20 languages: English, German, French, Italian, Spanish, Japanese, Chinese (Simplified and Traditional), Korean, Portuguese (Brazil), Russian, Arabic, Dutch, Turkish, Polish, Swedish, Hindi, Indonesian, Thai and Vietnamese. WHAT THROB IS NOT Throb does not diagnose, predict or treat migraine and is not a medical device. It records what you log so that you and your clinician can see the pattern. No streaks, no gamification, no community feed, no notification spam.
Migraine Tracker

Log a migraine in two taps. See what sets it off. Take a report to your doctor with a subscription. Migraine Tracker is a headache diary built to be used mid-attack, when reading a long form is the last thing you want. Start an attack while it is happening, mark it over when it passes, and fill in the details later if you feel like it. WHAT YOU CAN RECORD - How bad it is, on a scale you move with one thumb - Where it hurts, on a map of twenty areas of the head - Migraine, or another kind of headache - Aura, nausea, light and sound sensitivity, and more - What you think set it off - Which medication you took, and whether it helped - A note in your own words YOUR MONTH AT A GLANCE Home shows this week as a strip, with a dot on every day you logged. The month card counts your migraine days and splits them into mild, moderate and severe, so a bad month is visible without counting anything. WHAT YOUR DIARY SHOWS The Insights tab opens on the month as a calendar, then reads the last four weeks, three months, year, or everything: how many attacks, how long they lasted, which weekday and which time of day they favour, which medication helped most often, and how all of it compares with the stretch before. THE REPORT YOUR NEUROLOGIST ASKED FOR (SUBSCRIPTION) With a subscription, one tap builds a report of the last three months: the month calendars, a table of every attack, your medication, and your MIDAS score. Save it as a PDF or send it as plain text. AIR PRESSURE FORECAST AND ALERTS (SUBSCRIPTION) A falling barometer is one of the most reported migraine triggers. With a subscription, the app charts the pressure for the week ahead where you are, can warn you the morning before a drop, and counts how many of your own migraines began within a day of one, so you can see whether it is a trigger for you rather than assume. The current reading from your phone's barometer is included. NO ACCOUNT, NO CLOUD No account. Your diary stays on your phone. Export a backup file whenever you want and open it as a spreadsheet. Logging, the calendar and the insights are included. The doctor report and the pressure forecast and alerts require a subscription, which you can cancel at any time. This diary is not a diagnosis. Verdict
Throb and Migraine Tracker are too close to call on the numbers: across 5 compared parameters — price, ads, in-app purchases and device support among them — neither pulls far enough ahead to decide it. What separates them is feature set and interface, not measurable difference.
Scored on Price · Rating · Positive reviews · Number of ratings · Update frequency · Ads · In-app purchases · Monetization · Best chart rank · Devices · Requires iOS
Both are free to download. Throb sells 1 one-off in-app purchase and Migraine Tracker sells 2 in-app purchases, starting at $4.99 per month. Whichever you choose, the download price is not the number that matters.
