Your medical history lives in a dozen places — this practice, that hospital, a folder in a drawer. Matriarch Care puts all of it in one place you actually control. Built by a nurse practitioner with more than twenty years in patient care, for the people who spend their lives explaining their health to someone new. A MEDICAL ID THAT ANSWERS THE FIRST QUESTION Allergies, conditions, current medications, blood type and emergency contact sit at the top of your records — ready the moment a doctor, a nurse or a paramedic asks. EVERY DOCUMENT WHERE YOU CAN FIND IT Lab results, imaging, prescriptions, visit notes, insurance cards and advance directives sort into folders you name. Photograph a paper record or upload a PDF and it goes where it belongs. MEDICATIONS, CURRENT Dose, frequency, prescriber and start date for every medication you take. Open the list at the pharmacy counter instead of trying to remember. VITALS OVER TIME Log blood pressure, blood glucose and weight, and see the trend — not just today's number. SHARE IT, THEN TAKE IT BACK Send a doctor a secure link to your health summary or a specific folder. Set what they can see, set when it expires, and revoke it whenever you want. Show the QR code at the front desk instead of filling out the clipboard again. FOR THE PERSON MANAGING A PARENT'S CARE Give a family member access to the records they need to help. They see what you allow — nothing more. BUILT FOR PRIVACY Your records are yours. Encrypted storage, no selling of your data, and no sharing with anyone unless you create the link yourself. WHO IT'S FOR — Anyone seeing more than one doctor or specialist — People managing a chronic or ongoing condition — Adult children coordinating a parent's care — Anyone tired of repeating the same tests because the records did not arrive Matriarch Care is a personal health record. It does not provide medical advice, diagnosis or treatment. Always talk to a qualified provider about your care. Hello Joan is a pilot maternal mental health screening tool. Instead of filling out a clinical questionnaire, you have a brief conversation about how you have been feeling. Joan uses that conversation to generate three validated screening scores (EPDS, PHQ-9, GAD-7) and a clinical brief you can share with your healthcare provider. Screening takes about three to five minutes. Everything is anonymous. WHAT YOU GET Your validated screening scores across the three most-used perinatal mental health instruments. A clinical brief written in language your provider will recognize. Option to save the summary as a PDF or print it and bring it to your next appointment. WHO IT IS FOR Pregnant women, new mothers, and anyone in the perinatal period who wants a clearer picture of how they have been doing. This includes the first year postpartum. HOW IT WORKS Tap Begin, review the consent notice, and start the conversation. Joan will ask a few clarifying questions the way a thoughtful clinician would. When the conversation ends, Joan generates your screening summary in about ten to twenty seconds. WHAT WE DO NOT DO Hello Joan is a pilot screening tool, not a substitute for clinical diagnosis or treatment. Your conversation is processed to generate your result and is not stored on our servers after your session ends. IF YOU ARE IN CRISIS Call or text 988 (Suicide and Crisis Lifeline). Text HOME to 741741 (Crisis Text Line). These resources are one tap away in the app at all times. Your story is more than just a number.
