Verdict
The clearest difference is review sentiment: Dr. Solitaire at 92% against Rummy's 47%. Dr. Solitaire also leads on monetization model (ad-supported vs free with in-app purchases). Rummy's advantage is rating (3.9 vs 3.0) and ratings volume (1.5K vs 2). On price, ads, in-app purchases and device support there is nothing between them.

Both are free to download. Neither carries in-app purchases, so what you see is what you pay.
Rummy holds the better App Store score, 3.9 against 3.0. That said, the two scores are not equally well evidenced: Rummy has 1.5K ratings behind it against 2, so its average is the more settled of the two. Written reviews complicate that: 47% of Rummy's written reviews are positive against 92% of Dr. Solitaire's, so the app with the better star average is not the one people write more warmly about.
Rummy ships an update every 3 months, Dr. Solitaire every 6 months. The most recent releases landed on October 18, 2026 and October 1, 2026 respectively.
