The Care Team can be made up of doctors, doctor’s staff, the patient or family, Pharmacy etc. Hubchart’s Patient Centric Groups are especially valuable for Complex Medical Care, Case Management and Concierge Medicine where individualized care can be delivered by a multidisciplinary team continuously coordinating with each other to provide optimum care.
