Make the most out of your healthcare appointments, make your care safer, know where to go for help and what questions to ask, record your medication and personal health information make notes and have everything securely in one place with this easy-to-use FREE app developed with doctors, pharmacists and patients. It can also be used if you get help, or help look after, a family member or friend. You can use this app to • Identify where to go for help • Useful information and tips-plan what you want to discuss in a consultation with your doctor, nurse or pharmacist • Make notes after an appointment • Keep a diary to record notes and information • Prompts of questions to ask for planning before a consultation, using remote consultations, and going into and out of hospital, as well as patient safety information (eg what are my rights as a patient). This is important because: • It will help you get the most out of your appointment • Know what to do if something changes • Support you to identify different care and health management options • Can be shared with trusted people to support managing your health or others • Help to store your health information in one convenient place that you always have with you This app has been developed by the NIHR Greater Manchester Patient Safety Translational Research Centre. We have co-developed this guide with a lot of stakeholder involvement and engagement activities with patients, carers, members of the public, GPs and pharmacists. The guide aims to support patients and carers to be more actively involved in their patient safety in primary care which is a key national and international priority for patient safety and care. It is also available as a paper booklet, and a website (https://www.patientsafetyguide.org/). The Guide app is free to download and use. It does not provide medical advice, diagnosis or treatment. Always seek the advice of your physician or other qualified health provider with any questions about your health, medical conditions or medications. This does not replace your medical record.
