It is developed by scientists and physicians from Johns Hopkins Medicine, Kaiser Permanente, and Palo Alto Medical Foundation. The app enables your doctor to remotely monitor your health status daily and helps detect your health risks early. When important care moments are identified, your doctors are notified remotely for early care intervention. The app matches you with a dedicated health coach remotely. Your health coach designs a personalized care plan for your health goals. Through regular sessions with your coach, you are supported every step of the way to make positive changes towards a better you.
