The mySomatus app makes it easy to stay on top of your kidney health goals and connect with your care team. The app is available to members enrolled in the Somatus Kidney Care program, available through select health plans. With the mySomatus app you can: - View all of your upcoming scheduled care team visits - Watch short on-demand educational videos - Track progress against kidney health goals - Manage and view daily, weekly, and monthly activities The mySomatus app is meant to empower you to take control of your kidney health! --------------------------------------------------------- The mySomatus app is free for eligible members of partnering health plans.
