You've made me very happy.
I'm so glad I found this app! I used to use Bento to keep track of sooo many things and was devastated when it was discontinued. I tried a wide variety of note, database, & inventory apps, but nothing really worked for me. Then I found FormConnect. I love the book and/or clipboard style and the ability to create books/files to keep track of various things regarding which I need different information.
I would give it 5 stars if it had the following features:
1. I really HATE the background on the initial screen where the files/books are displayed and would LOVE to be able to substitute a solid background.
2. I would love to be able to use a wider variety of book and paper colors & to choose which paper goes w/ which book cover or clipboard.
3. I want to be able to rearrange the books/files on the initial screen.
4. Within a book or file, I want to be able to swipe from right to left or from left to right to move from one record to the next without using the list on the left, but want to keep the list on the left to navigate quickly to a given record.
5. I wish there were a basic help system to answer questions rather than having to figure things for myself.
6. When you first choose the app, you get a screen which may provide good info on using the app, but you get a succession of pop up boxes that ask whether or not to deal w/ a variety of files about which you have no knowledge. This needs to be fixed.Show lessDr.Memmie
Very helpful, could use some improvements
As an environmental scientist, I use this app for collecting data in the field. I basically created versions of my own standard forms in the app and then I can export the data via a csv. File can manipulate the data in excel. What I would like to see is the ability to change back to "no selection" in a drop down tab in case I accidentally hit the wrong button and the ability to tag data to a GPS location. Also, if they added the ability to create/edit forms outside of the app on a PC/Mac , this would be a 5 star. It takes a little bit of time to set up your form the way you want it, but after you do it is incredibly easy to manipulate. Good app but could use some additional "user friendly" features
I like the ability to create forms based on my needs. I upgraded a year ago to formconnect pro. I keep having problems with the app crashing in the middle of use with clients - this is extremely inconvenient and embarassing. PLEASE FIX THIS GLITCH. Customer service could be friendlier - I wish there was an option for providing suggestions. I've emailed suggestions and have received defensive emails in response. I would recommend allowing suggestions or surveying users for needs. I'd like to see the ability to select all and change font size or characteristics for text that has already been entered. I'd also like to be able to either "hide" templates that I don't currently need or arrange templates in a preferred way instead of it automatically going alphabetical. Finally, it would be great if a specific text field could be set to carry over into multiple pages (e.g. customer name) and if pages numbers could be added (e.g. page x of y).Show lessPacifica_girl
Well thought out
This app is very well designed. Provides great flexibility and a great feature set. I am using it to create commissioning check sheets for a power dam. A joy to use.
1 bug: if you add a field to a form that already has records, the field will populate with data from an existing field and blank the existing field.
1 wish: some way to flag records on the record sidebar so I know which records are complete.

wasted my time & money -_-
I was excited to use this app for my business. I have clients that have to fill out waivers & release forms for services & was tired of printing out the forms. I thought it would be great to be able to have an electronic version that I could send my clients prior to their appointment so they could fill them out & sign them & return back to me. Saving time & paper. The description says "be able to share with others". What it doesn't say is that the "others" must have an iPad & FormConnect on their iPad to be able to complete it. Very disappointed!! Also the customer service response when I asked a few questions was very poor. Basically a one sentence reply with best regards, then the reps name. If you need this app for anything like I did, look elsewhere do not waste your time or money. All my normal forms are in word format & there is not a away to import these to formconnect. You HAVE to start from scratch creating fields, labels, text the entire form has to be designed.Show lessdce012014
Formconnections
Needed suitable app for form input of company data instead of paper versions. After looking at lots of other apps we settled on FormConnect and are very pleased with its functionality and versatility to fit in with existing document apps we use and Dropbox. Very quick response to queries I had both before and after purchase. Highly recommended for anyone that uses forms.Show lessDavenort
Used to be excellent....
I've had this for a long while and found it really good. However having just upgraded to the latest version I have two complaints. The record scrolling arrows have been removed and when moving through records the list keeps recentering which makes it less easy to scroll. Secondly, and this is a major degradation, the performance in moving through a record set is painfully slow. I have a database of some 500 single page records, each with a single image and 10 text fields. Using the older version moving from one record to the next was almost immediate. With the latest version it is taking between 5-10 seconds for the next record to be displayed. Coupled with the change to how the record set is moved through, this is a major disappointment. Had I known I would not have upgraded. The App Store images are also misleading as they show the old record layout with the scroll arrows!!!Show lessMoray99
Good, but...
This app has great potential, but for me it's currently marred by frequent crashes though, to be fair, I've not suffered any data loss so far.
I've also found that the location finder remains live and doesn't hold data if the form is multi-page. Move to another page, then return and your location data has disappeared.
Another pet peeve is the manner in which images have to be loaded in to their containers. Forget copy and paste: the only way to insert an image is to first save it to Photos, and then select it from the container popup menu. It's a tiresome process when you're dealing with several images per record.
For simple text data collection though, it's definitely up to the task.Show lessMacJudd
Top Notch App
This is easily the best form/database out there in terms of layout and creating a real form that guides users through the process of adding data ... And that is soooo important.
It's really easy to use ... Although there is room for improvement of course, but no way I'm taking stars off for that. As is, totally useable, flexible, and for one issue I had, the developer was lighting fast with a response and fix. I was very impressed.
I bought the Pro update ... The Kisok mode is great if you don't want users to have access to data records... Or need calculations in the form. Global or selected records, export to csv, PDF , and form backup via DropBox and email meets my needs entirely.
Sure, I'd like email fields which, when populated and tapped, open a ready to go email, export of a selection of fields within the form ( rather than the whole form ), a 'Next' key on the keyboard to jump to the next text field, maybe an export directly to Numbers on the iPad ( or iCloud)... but these are just conveniences which don't impact on the thrust of the app’s purpose.
I'll email the dev with more ( probably unreasonable ) requests!

Need more for $$$
Would give 5 stars if it had the ability to be opened in Word. Clients love signing the contract on the iPad, but can't open questionnaires without converting from PDF and not everyone can do that.
Would like to be able to have different fonts though so we can better reflect our image. Also wouldn't mind having a scale for our post event questionnaires. A few more backgrounds or the ability to create your own might be a good idea. I'd like to have each of our questionnaires and surveys with a different "cover" that includes our logo.Show lessMetro DC DJs
