About

A letter from the hospital in your hand, and no idea what it says. An appointment in three days, and half the questions only come to you on the way home. Continuum is built for those moments. YOUR REPORTS IN PLAIN LANGUAGE Paste the text, photograph the page or upload the PDF. Continuum explains the terms, shows each value next to its reference range, and suggests questions worth asking. It does not diagnose. A BRIEF FOR THE CONVERSATION Your lab results, discharge letters and imaging reports become one ordered summary: timeline, medications, open items. Shareable as a PDF, in the language your doctor reads. PREPARED, NOT RUSHED Gather what you want to say before the appointment. The day before, the app reminds you, and afterwards there is room for what was actually said. YOUR STORY, IN ORDER Every brief, preparation and explanation joins one calm chronological view. You can see what happened, and when. YOUR DATA IS YOURS Briefs you keep offline are encrypted on the device. Face ID can lock the app. Signing out erases the local copies. A notification never carries health details, only that something is ready. SIX LANGUAGES German, English, French, Spanish, Persian and Arabic, with right-to-left script where it belongs. IMPORTANT Continuum is not medical advice and does not replace a conversation with a clinician. The app explains terms and helps you prepare. Decisions about your care are made together with a qualified professional. Privacy: https://continuumcare.app/privacy Terms: https://continuumcare.app/terms
