Não consigo me casastrar

Baixei pelo IOS e esta com erro.
Não consigo me cadastrar pq fica mandando eu por o sobrenome.
O campo nome quando eu preencho repete no lugar do sobrenome, e se eu apagar o campo sobrenome para por o sobrenome também apaga o campo nome.
Ai eu tento me cadastrar e não consigo pois diz que o campo sobrenome esta errado!
