About
HeartAge+ is the patient companion for cardiac care at PACE Cardiology.
It brings your heart health into one place: the readings you take at home, the data from your wearable, the plan your care team has set, and the things you want to remember to ask at your next visit.
WHAT YOU CAN DO
- See your Heart Age, a plain-language summary of your cardiovascular risk factors, alongside a biological age estimate
- Track blood pressure, weight and symptoms, and watch the trend rather than a single reading
- Connect a wearable - Apple Health, Oura, Whoop, Garmin, Fitbit, Polar and others - so heart rate, HRV, sleep and activity feed the same picture
- Follow your action plan: the specific steps your care team has recommended, with reminders
- Move & Breathe: guided breathing, meditation and coherence training that responds to your live heart rate when a compatible monitor is connected
- Keep a symptom journal, so what you felt and when is there at your appointment
- Message your care team and join telehealth visits
BUILT AROUND YOUR CARE TEAM, NOT INSTEAD OF IT
HeartAge+ is made for patients under the care of a cardiology practice. It does not diagnose and it does not replace medical advice. It helps you and your clinicians look at the same information between visits.
MEDICAL DISCLAIMER
HeartAge+ is not a substitute for professional medical advice, diagnosis or treatment. Always seek the advice of your physician or another qualified health provider with any questions about a medical condition, and before making any medical decisions - including decisions about medication, exercise or treatment - based on information shown in this app. Heart Age and the other figures in the app are informational estimates prepared for review with your care team; they are not a diagnosis. If you think you may be having a medical emergency, call 911 immediately.
YOUR PRIVACY
Your health information is stored in Canada and handled under PHIPA. It is never sold and never used for advertising.
An account from your clinic is required to sign in.

Food log: foods you add or scan now appear in their meal, and after scanning a barcode the app asks which meal to add it to. Oxygen overnight (iPhone): the card shows the running time and the seconds recorded as the night goes on. Blood pressure: the "Before you measure" checklist also shows while the app waits for your Bluetooth cuff.
