This hospice app provides a convenient way for healthcare providers to refer patients to the Hospice of New York. This app also provides information about clinical services provided by medical directors of the Hospice of New York. Healthcare providers will also find guidelines on hospice eligibility. Referrals submitted through the app will be received by the Hospice of New York's admissions office for expedient follow up. All information received through the referral form complies with and will be processed according to HIPAA regulations.
