1. 出院個管照顧服務，包括：基本照護與用藥等衛教資訊、門診預約
2. 記錄每日血壓、血糖、用藥及返家運動處方，如有異常，系統會通知提醒返院做進一步檢查
3. 健康促進服務，像是運動與營養建議
4. 健康課程安排，長者可藉由課程實作，落實在家安養
