Filing a kakutei shinkoku in Japan? The medical-expense deduction (iryohi kojo) applies once your household's yearly medical spending crosses 100,000 yen (or 5% of income), but while receipts pile up in a drawer, nobody can tell how close they are. Iryohi Counter is a purpose-built counter for exactly that line, and every recording feature is free.
How much to go, at a glance
Enter your total income and the app computes your deduction line, the smaller of 100,000 yen and 5% of income. A big gauge on the home screen answers how much so far and how much to go. Once you cross the line, it switches to your estimated deduction and refund at your marginal rate.
Two-tap logging
Enter the amount and save. The date defaults to today, the family member and category stay as last used, and the payee is one tap from your recent list. The more you log, the faster it gets.
Family first
The deduction is claimed per household, so entries are grouped by family member, matching the official statement's name-of-person-treated column.
Export the NTA format
Write out the Iryohi Kojo no Meisaisho five columns plus the A to G calculation block as an Excel-friendly CSV (UTF-8 BOM). Copy it straight onto the form or use it to check your e-Tax entry.
Your records stay on your iPhone
Medical amounts, payees, receipt photos and settings are stored only on your device. There is no developer server and your records are never sent anywhere.
Pricing and ads
The app is free to download, and medical records are unlimited and always free. To support development, a banner ad (Google AdMob) is shown at the bottom of the screen. If you prefer a cleaner view, an optional one-time Remove Ads purchase (100 yen, no subscription) hides it. Every feature stays the same after purchase; only the ad goes away.

This app only aggregates your payment records; it does not make tax judgments. Confirm eligibility, deduction and refund amounts with the NTA guidance or your tax office. All figures are estimates and exclude special reconstruction income tax and resident tax. The Self-Medication Taxation System is not supported. This app is not a medical device and is not intended for diagnosis or treatment.
