Crashes when I go to upload a photo to personal info

This app does not work.

Crashes as soon as you click continue on the first screen.

Doesn't work

Installed it. Got to the first page and it says to select organization but gives no drop down menu or any place to enter anything so I hit continue and it shuts the app down. Not useful if I can't get into it.
