Error al solicitar Reembolso

Al momento de solicitar un reembolso la App automáticamente coloca el número de afiliado del plan dental en vez del ejecutivo. Y al final cuando colocó los datos bancarios me da un error que dice número de afiliado incorrecto. Por favor corregirlo
