Mind Therapy Clinic's patient app is a companion for people already receiving care at Mind Therapy Clinic. It gives you one place to stay on top of the between-session parts of your treatment — your schedule, your check-in questionnaires, and the paperwork your care team needs. This app is for established Mind Therapy Clinic patients. What you can do: • See your schedule. View your upcoming group and program sessions, times, locations, and whether a session is in person or online. • Complete outcome questionnaires. Fill out the check-in measures your clinician assigns (such as the OQ-45.2 outcome questionnaire) directly in the app, so your care team can track how you're doing over time. • Handle paperwork and requests. Submit and track requests to your care team — for example leave-of-absence notices, records-release (ROI) forms, prescription and enrollment requests — and sign documents when needed. • Manage your profile. Review your contact details and, where available, subscribe to your session calendar. The app keeps your information private: you're signed out automatically after a period of inactivity, and your screen is hidden in the app switcher when you leave the app. This app is not for emergencies.
