This app has been updated by Apple to display the Apple Watch app icon. The text on the summary page (also called scores) was not being cleared correctly if the score was <5. This error has been fixed.

Good simple screener

Good screening tool for depression and anxiety. Only issue is that depression results are for PHQ-9, which has a maximum score of 27, but app lists it as 20. Other than that easily fixable error, it’s a great way to check a person’s progress in treatment.

Screening Tool

If you're already comfortable with your diagnoses of depression and/or anxiety disorder, you won't need to download this app. It consists of testing used in various online self-reporting screeners. The app calculates scores. Questions pertain to "the past 2 weeks" but it looks like results are dated -- possibly you could adapt results as a daily tracker, but it isn't what the test was developed for.
You have to pay to get the app that follows up screening results. I didn't buy.
