There's not much here for us to act on. SigmaMD isn't a Medicare app — it's provided through your clinic — so this sounds like a mix-up with a different app. If you've got a real issue, tell us what it is or reach out to your clinic and we'll help sort it out.

We're not aware of any keyboard issues on the sign-in screen, and it isn't something we're currently seeing on our end. If you're having trouble getting into the app, the fastest way to get help is to reach out to your provider's team — they can walk you through sign-in and make sure you have access.
Create an account

Nothing stupid about it — you just misread how it works. You don't self-register: your clinic sends an invitation to your email, and the code in that invite creates your account. No account needed first, no chicken-and-egg. If you want in, ask your clinic for an invitation.
Nightmares

Glad you got your labs in the end. The password prompts to download and the rating screen that "spun and spun" are Apple's, not ours — Apple Support (support.apple.com) is the right place to get those sorted out.
Can’t make sppointments

Appointment availability in the app is set by your clinic — if you're only seeing urgent care with no open times, it usually means your clinic hasn't enabled self-scheduling for other visit types yet. The best next step is to reach out to your clinic and ask them to open up availability; their team controls which visits and times patients can book.
