The Trium mobile application allows you to capture your vital signs in real time from various Bluetooth devices (blood pressure, pulse oximeter, digital scale and thermometer) for the purpose of remotely tracking certain vital signs and transmitting that data to your care team. The app is a part of an innovative health solution to provide high-touch support and education for people living with chronic diseases. Our programs are focused on improving member quality of life, reducing hospital utilization, and preventing the overall escalation of chronic diseases.
Our goal is to work with each patient to improve their quality of life while focusing on reducing the overall cost of care. The app helps manage complex chronic conditions more effectively while delivering cost savings.
