The Your Care Connectapp is available for patients at Ontario Shores Centre for Mental Health Sciences. Ontario Shores Centre for Mental Health Sciences is a public teaching hospital providing a range of specialized assessment and treatment services to those living with complex and serious mental illness. Patients benefit from a recovery-oriented environment of care built on compassion, inspiration and hope. The Your Care Connectapp aims to assist in the delivery of patient care by collecting general health information from wearable devices, such as sleep tracking, to determine how changes in health information may impact the patient's overall health and well-being.
