Docket® partners with public health to make it easier than ever to access personal and family immunization records directly from Alaska Department of Health, DC Health, Idaho Department of Health and Welfare, Maine Department of Health and Human Services, Minnesota Department of Health, Missouri Department of Health and Senior Services, New Jersey Department of Health, North Dakota Department of Health and Human Services, Utah Department of Health and Human Services, and Wyoming Department of Health. Track upcoming shots, review past immunizations, share your official immunization reports, and more. Residents of select jurisdictions can access a SMART® Health Card vaccine credential QR code to facilitate travel to places with COVID-19 restrictions. MTN HLTH is the companion app for members of MTN HLTH, a longevity and health optimization medical practice. It gives you one private place to manage your care between visits. With the MTN HLTH app you can: • Message your provider right from the app • Review your personalized wellness plan and current medications • Connect Apple Health so your care team can see your metrics • Complete check-ins before your visits so your provider is prepared • See your lab and bloodwork instructions and track your results • Book, reschedule, and join virtual or in-office visits • View your prescribed therapies • Manage your membership, payment methods, and billing Access requires an active MTN HLTH account.
