Docket® partners with public health to make it easier than ever to access personal and family immunization records directly from Alaska Department of Health, DC Health, Idaho Department of Health and Welfare, Maine Department of Health and Human Services, Minnesota Department of Health, Missouri Department of Health and Senior Services, New Jersey Department of Health, North Dakota Department of Health and Human Services, Utah Department of Health and Human Services, and Wyoming Department of Health. Track upcoming shots, review past immunizations, share your official immunization reports, and more. Residents of select jurisdictions can access a SMART® Health Card vaccine credential QR code to facilitate travel to places with COVID-19 restrictions. The Horizon Blue app makes it easy to access your health plan information. Plus you can find care and connect with Horizon member services. The app is intended for use by Horizon members. Horizon Blue Cross Blue Shield of New Jersey is an independent licensee of the Blue Cross Blue Shield Association. The Horizon Blue app is free and lets you: • View your benefits and coverage details. • Visit with a doctor • View, download and share your ID card • Find in-network doctors, hospitals and other health care professionals • Track your out-of-pocket costs • Submit medical claims
