The Horizon Blue app makes it easy to access your health plan information. Plus you can find care and connect with Horizon member services. The app is intended for use by Horizon members. Horizon Blue Cross Blue Shield of New Jersey is an independent licensee of the Blue Cross Blue Shield Association. The Horizon Blue app is free and lets you: • View your benefits and coverage details. • Visit with a doctor • View, download and share your ID card • Find in-network doctors, hospitals and other health care professionals • Track your out-of-pocket costs • Submit medical claims
