Verdict
The clearest difference is ratings volume: MedHub at 5K against SIR Guidelines's 23. SIR Guidelines's advantage is update cadence (every 3 weeks vs every 3 months) and rating (5.0 vs 4.7). On price, ads, in-app purchases and monetization model there is nothing between them.

Both are free to download. Neither carries in-app purchases, so what you see is what you pay.
SIR Guidelines holds the better App Store score, 5.0 against 4.7. That said, the two scores are not equally well evidenced: MedHub has 5K ratings behind it against 23, so its average is the more settled of the two.
MedHub ships an update every 3 months, SIR Guidelines every 3 weeks. The most recent releases landed on October 29, 2026 and October 24, 2026 respectively.
