An Employee Contact page to choose which employee to review/evaluate. 2. Employee evaluation form. A Calendar page used to display which employees got evaluated on which day. Selecting a calendar day will be used to pull up the evaluation page. 4. Setup pages. This is where a reminder notification can be configured, or the start of the work week can be configured for the calendar, and the Evaluation Criteria will be set up. 5. Reporting is a summary page to allow the user to select the range of dates to see the evaluation results for each employee.
