FND Tracker is a private symptom diary for people living with Functional Neurological Disorder, built for days when logging anything at all feels like too much. Open the app and see a today-at-a-glance screen with your overall symptom load and a rolling seven-day trend. No scrolling through menus, no setup required each morning. Logging is a calm, simple checklist. Twenty-three common FND symptoms are organised into five groups - movement and balance, episodes, thinking, speech and swallowing, sensory changes, and pain, energy and sleep - so you can find what you need even on a foggy day. Tick what you are experiencing and give each symptom its own score from 1 to 10, so a bad tremor and a mild headache never have to share one number. Your overall severity for the entry is simply your highest score. Log your vitals too. Record heart rate and blood pressure on their own, separate from your symptoms, so readings never add to your daily symptom load. They appear on your Today screen and day details, and can be included in your reports. Good days count too. If nothing is wrong, record a clear day, and it shows in your calendar and your exported reports, so a day with no symptoms never looks the same as a day you simply did not log. Hospital stays matter. Record an admission with its length, reason and severity, kept with the day it began. Missed work days are tracked separately with duration, reason and severity, all viewable in a month calendar you can switch between symptom and missed work views. Together this gives you a clear, dated record of how FND has affected your life and your ability to work. Keep doctor's letters where you can actually find them. Import PDFs from Files or photos of letters into a built-in viewer, so every appointment, diagnosis note and referral lives in one place alongside your symptom history. When it is time for a neurology appointment or a conversation with an employer, export a clinical, paginated PDF or CSV for any date range, with the sections you choose - including hospital stays and vitals. Turn weeks of lived experience into something clear, organised and easy for someone else to review. Everything is protected with optional Face ID, Touch ID or passcode lock that re-arms whenever the app goes to the background. There is no account to create, no server, and no analytics. Your records never leave your device unless you choose to export them. FND Tracker is a personal record-keeping tool. It does not diagnose, monitor or treat any condition, and it is not a substitute for medical care. It simply helps you keep an honest, private record of what you are living with, so you have something solid to bring to the people who need to understand it. Download FND Tracker and start building a record that reflects what your days are actually like.

About

Baseline

Features

When pain changes from day to day, it can be hard to remember what happened by the time you see a doctor. Baseline helps you keep a simple record and bring a clear summary to the appointment. LOG WHAT YOU FEEL Start with a pain score from 0 to 10. Add body areas, what the pain felt like, a note, or an activity it made difficult when those details matter. You can also log a score on Apple Watch; it transfers to the paired iPhone when the devices connect. NOTICE PATTERNS IN YOUR OWN RECORD See how your scores changed over time, when you logged higher pain, and how often pain affected everyday activities. Insights show how much data each figure uses. Medication views show the doses you recorded and pain scores logged around them; they cannot tell you whether a medicine caused a change. BRING A READABLE REPORT Make a one-page PDF for a selected period, with a chart, a summary of what you entered, medication records, and questions you want to ask. Long details may be shortened to fit. Share or print the PDF from your device. REMINDERS WHEN YOU WANT THEM Set an optional daily prompt to log and reminders for medicines you add. Both start off. You can also mark a scheduled dose as taken from its reminder. PRIVATE BY DESIGN Your health record stays on your device. Apple Watch entries transfer to its paired iPhone; iPhone and iPad records do not sync. No account is needed, and we do not receive your entries. An optional app lock uses Face ID, Touch ID or your passcode. A PDF is a readable copy, but Baseline cannot import it or restore your record from it. Save one before changing devices or deleting the app if you want a copy to read later. FREE TO START; PLUS FOR LONGER VIEWS Logging, reminders, Apple Watch, the app lock, PDF reports for one month, and insights for the last 30 days are free. Baseline Plus adds 90-day and year insights, and reports covering 3, 6 or 12 months. Choose a monthly or yearly subscription. If Plus ends, your entries remain on that device; the longer views require Plus again. Subscriptions renew automatically unless cancelled at least 24 hours before the current period ends. Payment is charged to your Apple Account when you confirm a purchase; renewals are charged within the 24 hours before the period ends. Manage or cancel a plan in your Apple Account settings. Baseline describes what you enter. It does not diagnose a condition or recommend a treatment or medication change. Talk with a qualified clinician before making medical decisions.
